If you have Medicaid, you should bring...

  • Proof of income (child support, SSI, pay check stubs, food stamps)
  • Proof of residency - anything with current address (bill, rent receipt)
  • Medicaid card for the current month
  • Any other insurance cards

    If you have insurance, you should bring...

  • Insurance card
  • Driver's License
  • Co-pay

    If you do not have insurance, bring at least $70 to cover the intake (see rates). Let our billing specialists help make arrangements for payment.
