What To Expect

When you call our office, the receptionist will ask for your basic information and insurance, so please have it handy. A referral form is completed over the phone, and you will be asked about your situation so we can match you with the best therapy service for your needs.

Family Life Counseling accepts a variety of insurance plans, and as a service to you, we will contact your insurance company prior to scheduling your appointment so you will know what your co-pay will be. We are also a Medicaid, Medicare, and Workers' Compensation provider.

You will be scheduled to come into the office 30 minutes before your appointment to complete intake paperwork. Please bring your driver?s license/state ID, insurance card, and co-pay. Co-pays are due at the time of service.


Fees for Services:

Diagnostic Assessment and Testing $132 (first session only)
Individual, Family, Marital Counseling            $90 per hour
Group Counseling $40 per hour
Case Management $85 per hour
Crisis Intervention Services $150 per hour
Psychological Evaluation Testing $600 per hour (includes scoring &
report, not covered by most insurances)
Missed Appointments or Cancellations $35
